+ What is the Bell Health & Wellness Center?

The Bell Health & Wellness Center provides eligible employees and their eligible dependents convenient access to comprehensive healthcare and wellness services. The Center is focused on primary and preventive care as well as occupational health needs.


+ Who is eligible to use the Bell Health & Wellness Center?

Premise Health will provide occupational care to all of our employees. It will also provide primary care services to active employees, under age 65, for personal medical care. The Center will also be accessible to eligible dependents age two (2) and up as long as they are covered on Bell’s insurance through UnitedHealthcare.


+ Where is the Center located?

The Bell Health & Wellness Center and Pharmacy are conveniently located on the Ft. Worth campus at 720 E. Hurst Blvd., Hurst, TX 76053. It is easily accessible by employees and dependents with dedicated parking and is handicap accessible.


+ What are the Center’s hours of operation?

Monday – Friday, 7 a.m. – 5 p.m.


+ How can I make an appointment at the Wellness Center?

You can make personal medical appointments in a variety of ways:

You can visit your online patient portal, My Premise Health, where you can make your appointments via your computer, phone or tablet. Visit mypremisehealth.com once you have an active account.

You can also call 817-280-2188 or visit during regular operating hours to make an appointment.

If you are experiencing a medical emergency please dial 9-1-1.


+ Does the Center accept walk-ins?

The Center will gladly welcome walk-in patients, but the preferred method is scheduling an appointment. In most cases, same-day appointments will be available.


+ What types of healthcare services does the Center offer?

The Center offers a variety of services including primary care, acute care, and other services. For a full list, visit the services page.


+ Does the Center take my insurance?

For personal medical care, Premise Health is considered an in-network medical provider for Bell’s insurance through UnitedHealthcare. As with any provider, deductibles and co-insurance will apply based on the service and the benefit offered. For non-Bell medical plan insurance, Premise Health may be considered in-network, but employees should check with their insurance carrier and file claims for appropriate reimbursement; however, preventive care is covered at 100% with no co-pay.


+ Do I have to use the Bell Health & Wellness Center?

No, receiving personal medical care at the Bell Health & Wellness Center is completely voluntary. The choice is yours. You can continue to receive care in the community if you desire, but the Center is located on the Ft. Worth campus as an option for your convenience. If you choose to receive primary care services at the Bell Health & Wellness Center but need a referral to a specialist, the onsite team can help connect you to the appropriate specialist for your health plan.


+ I currently have a Primary Care Provider (PCP). Can my medical records be shared between the Center and my PCP?

You can request that your PCP share your records with the Bell Health & Wellness Center by completing your provider’s record sharing process. Additionally, you may request your records from Premise Health be shared with your PCP or specialist.


+ How will my current medical provider know I am receiving care at the Center?

After you have been seen at the Center, you may request any records to be sent to your community provider. If your community provider uses Epic as their electronic health record (EHR) system, your records will be available via Epic’s Care Everywhere and vice versa.


+ Is the Bell Health & Wellness Center operated by Bell employees?

No. The Center is staffed and operated by Premise Health. Premise Health manages the Center just as any licensed medical office in the community.


+ What type of medical staff works at the Center?

Premise Health has recruited a high-quality team of dedicated healthcare professionals to staff and manage the Bell Health & Wellness Center. The team includes a physician, nurse practitioner, registered nurse, a center manager and other support staff.


+ Is there be a fee for services at the Center?

Yes. Members will be charged $50* per encounter. For those on the Bell insurance through UnitedHealthcare, the fee will be applied to your deductible and/or co-insurance. For those not on the Bell insurance, you will be offered a bill at the end of your visit that you can submit to your insurance and will be subject to reimbursement or adjudication based on your plan specifics.

Preventative services, like annual wellness exams, are offered free of charge.

*There could be exceptions to the encounter rate in certain circumstances (e.g. specialty labs). You should be notified at the time of service if additional charges will apply.


+ How can I pay for my visit?

Payment is expected at the time of service. We accept credit, debit or HSA cards.


+ What services apply to my deductible?

Services that are non-preventive care and non-occupational health will apply to your deductible. For example, sick visits, non-work related injuries, and labs, will be billed to insurance and applied to your deductible if you are covered under Bell’s insurance, UHC.


+ Why can’t those 65 years and over use the Center?

Due to the governmental requirements it was not an option at this time.


+ Can I get prescriptions filled at the Center?

Yes, the Center will have a full‑service pharmacy. The pharmacy is an independent pharmacy under the Bell insurance. The amount you pay will be the same as if you went to any other in-network pharmacy and will be applied to your deductible and co-insurance. In addition to prescriptions, you will also be able to purchase some over-the-counter medication and supplies at the Center.

+ What is My Premise Health and how can I register?

My Premise Health is your new member portal for personal medical care. My Premise Health is a secure online and mobile tool that will put your health information in your hands. Visit the member portal page to learn more about what My Premise Health can do for you and how to register.


+ Will my health data be secure? Will anyone at Bell have access to my personal health information?

Your personal health information will remain confidential and your data will be kept secure. The Center will operate in accordance with HIPAA and will work diligently to protect all health records. Premise Health can’t share any personal health information without your permission unless it pertains directly to an occupational health related issue. Notice of privacy practices are available for review within the Wellness Center.
